District Nursing Services
The District Nursing Team assess, prescribe and deliver skilled nursing care to sustain patients in their own homes or in residential accommodation. Their aim is to promote and teach self-care whenever possible leading to personal independence.
District Nurse Teams are led by a District Nurse who is specially qualified to assess your individual needs.
The team includes qualified nurses and trained Health Care Assistants. You may also receive care from nurses in training under the guidance and supervision of your District Nurse.
Our District Nursing Team
Our Nursing Team is based at Canolfan Goffa Ffestiniog and work closely with the practice. They provide continuous care to people of all ages in the community as well as the residential homes in Llan Ffestiniog.
The Nursing Team run a Tissue Viability Clinic which is held in the Health Centre each week day from 11:30 to 12:30 p.m. by appointment. You can contact them on 01766 832 573.
Also based at Canolfan Goffa Ffestiniog are a Chronic Disease Case Manager and the Intermediate Care Team – both can be contacted on 01766 830 225.
